How the Nomination Form works
The nomination form is customized for each new class of Fellows and can be managed through the specific class record on Salesforce. Click here to learn more about the Class record.
Nomination Form - Base
This is the default form if there are no parameters to identify the program/class for the nomination. This URL should not be used on its own.
Nomination Form - Class
By appending the Class ID to the base form URL, Form Assembly is able to identify the class to which candidates will be nominated and pull that information into the form. [Reference: Custom Links in Form Assembly]
Review the header (program logo, title, intro text) and any custom questions (both those which are additional to the base form and those which should not be included) as well as the seminar dates for the class (under Fellowship Commitments) for accuracy.

Nomination Form - Personalized
The nomination form can be further personalized by appending a Contact ID to the form URL, using "cid=" [Reference: Custom Links in Form Assembly]
